Established in 2002 from parishes of Kollam, Niranam and Chengannur, Mavelikara is the site of the Puthiyakavu Cathedral and the 1836 Mavelikara Padiyola.

The region is historically significant for the Church. It holds St. Mary's Orthodox Cathedral (Puthiyakavu Pally), founded in 943 CE, which hosted the Mavelikara Padiyola of 1836, in which the Malankara Church resisted British anglicisation.
The diocese was established on 10 August 2002 by decision of the Holy Episcopal Synod, announced by Kalpana No. 134/2002 of Catholicos Baselios Mar Thoma Mathews II, and formed from 41 parishes and 12 chapels of the Kollam, Niranam and Chengannur dioceses. Kolathukalathil Paulose Mar Pachomios was appointed first Metropolitan and guided its initial development. Land was acquired at Thazhakkara for a permanent headquarters, the Theobhavan Aramana, consecrated on 17 October 2008 by Catholicos Baselios Mar Thoma Didymos I.
After Mar Pachomios's untimely death in August 2012, the Supreme Heads of the Church, Baselios Mar Thoma Paulose II and Baselios Marthoma Mathews III, served as Metropolitan, assisted successively by Joshua Mar Nicodimos, Alexios Mar Eusebius and finally Abraham Mar Epiphanios, who currently serves as Metropolitan. The diocese oversees dozens of parishes and chapels and humanitarian institutions such as the Paulose Mar Pachomios Shalem Bhavan for mental health care and the Cheppad Mar Dionysious Old Age Home.
