Created in 2002 from the historic Diocese of Kandanad, Kandanad East comprises parishes with a majority of the schismatic faction and is headquartered at Muvattupuzha.

The unified Diocese of Kandanad was established at the Synod of Veliyanad in 1877, under Patriarch Ignatius Peter IV, as one of the seven foundational dioceses of the Church. Its territory centred on ancient places of worship such as Kandanad St. Mary's Church and landmarks near Udayamperoor, and covered parts of the present Ernakulam, Kottayam and Idukki districts. Its first Metropolitan, Paulose Mar Ivanios, later became the first Catholicos of the East, Baselios Paulose I. Over the following decades, factional disputes within the Malankara Church produced overlapping jurisdictions, shifting allegiances and prolonged litigation, until a landmark Supreme Court verdict brought institutional clarity.
On the recommendation of the Malankara Association Managing Committee meeting at Parumala, the territory was divided on 20 March 2002 into the dioceses of Kandanad West and Kandanad East. Kandanad East, extending from Vaikom in the west to Idukki in the east, was placed under H.G. Dr. Thomas Mar Athanasius and comprises parishes where the schismatic faction holds the majority. It is headquartered at the Bishop's House, Muvattupuzha, and oversees dozens of parishes, chapels and a cathedral.
